How to Receive a COVID-19 Vaccine

The vaccine is available to ALL Los Angeles County residents 16 years or older*. The vaccine is provided at no cost for everyone regardless of immigration status or insurance coverage.

*Minors must be accompanied by a parent or guardian to consent for vaccination.

The City of Los Angeles currently runs ten fixed vaccination sites. City walk-up and mobile locations are now offering the vaccine without an appointment though you can still plan ahead and pre-register for a vaccine by visiting the City of L.A. Vaccination Portal

Appointments are still required for the City’s drive-thru sites. You can make an appointment by visiting Coronavirus.LACity.org/VaxReady or by calling 1-833-540-0473.

Other ways to book an appointment:

  • Speak with your healthcare provider
  • Visit VaccinateLACounty.com, L.A. County’s provider site, which lists all providers in the entire county.
  • Individuals who do not have internet access or who have a disability can call 1-833-540-0473 between 8 a.m. - 8:30 p.m. for assistance making an appointment. 
  • Visit VaccinateCA.com to search for providers by zip code.

Required Documentation for Vaccination Appointments

In order to be vaccinated, you will need to show:

  1. Photo ID and
  2. Proof that you are age 16 or older

You do not need to show government-issued ID and you do not need to be a US citizen to get a vaccine. Below, is a list of documents that will be accepted. This list is not complete and other documents may be accepted. One document may meet more than one requirement (e.g. a driving license).

If you have health insurance, please bring your health insurance card (COVID-19 vaccines are free regardless of insurance status)

Consent for minors: Minors (age 16 and 17) who meet the eligibility criteria can only receive the Pfizer vaccine. Minors must be accompanied by a parent or guardian who can provide consent for vaccination. If this is not possible, and you are attending a site run by Public Health, please bring a consent form signed by your parent or legal guardian. Required Documentation for Persons 16 or older

In order to be vaccinated, you will need to show:

  • Photo ID and
  • Proof that you are age 16 or older

Examples of proof of age:

  • Driving license or permit (foreign country or expired ID okay)
  • California ID card or REAL ID card (from the DMV)
  • Consular ID (Matricula Consular)
  • Medicare card
  • Military ID
  • Passport (foreign country or expired okay)
  • Birth certificate
  • Medical document/records from medical provider, clinic, or doctor
  • Membership card (foreign country okay if written in English)
  • Any official document that includes name and date of birth for example, school record

Second doses only

  • White vaccine record card or electronic vaccine record AND a photo ID

City of Los Angeles Vaccination Site Second Doses 

Note: Patients should note that the date stamped on the CDC vaccination cards received after their first dose is not an appointment confirmation. Instead, everyone should use the appointment details provided by Carbon Health.

If you received your first dose at City of Los Angeles Vaccination Site, you are guaranteed a second dose appointment within the CDC recommended time frame.

  • You will receive an email and text message within 48 hours after your first dose appointment with a time, date, and location for your second dose appointment. 
  • These second dose appointments will be automatically booked for approximately 28 or 21 days after your first dose appointment, depending on the vaccine you received
  • You do have the option to reschedule your Second Dose appointment, but it is recommended that you keep the slot you are given.
